This code is relatively old in that is requires the Perl module Nagios::Plugin. Back in 2015 that module was moved to the Nagios::Monitoring::Plugin name space. You will need to update the plugin to "Use" the new name space.

The alerting seems rock solid, but I can not get the graphing working with the check_mysqld.php file as provided. I keep getting Undefined offset errors instead of graphs. What am I doing wrong?
